To much autoattack with nevermore also bad farming given the fact that you had +50 dmg at level 5.
No shadowraze ? why ?, no hp regen after second trip to fountain, why ?, wraith bands? you had 777 hp. Also treads? i`m sorry but next time try this :

- improve last hit/deny ( you have 100 dmg )
- hp boost if you cant farm very fast your dagger/lothar ( bracers should do the job, or even ogre axe, because you'll need bkb later in the game)
- definitely not treads.
